Inhibits bacterial protein synthesis by binding 23S rRNA of 50S ribosomal subunit — prevents 70S initiation complex formation. Unique mechanism with no cross-resistance to other antibiotic classes.

Common Side Effects
Myelosuppression (thrombocytopenia, anemia — CBC monitoring mandatory), peripheral and optic neuropathy with prolonged use, lactic acidosis, serotonin syndrome.
Safety Warnings
Serotonin syndrome risk with SSRIs, SNRIs, MAOIs, tramadol, triptans — potentially fatal. CBC monitoring if use exceeds 2 weeks. MAO inhibitor properties — avoid tyramine-rich foods. Reserve for resistant infections. International shipping: 7–14 days
